Personal card T-2 is filled out for employees of all categories hired on the basis of an order (instruction) on hiring

The form of the personal card T-2 was approved by the Decree of the State Statistics Committee of Russia dated 04/06/01 No. 26 “On approval of unified forms of primary accounting documentation for recording labor and its payment (hereinafter referred to as the resolution of 04/06/01).

The personnel service employee must keep in mind that the rules for maintaining personal cards of the T-2 form, with the exception of the Instructions for the use and completion of primary accounting forms and documentation for labor accounting and its payment, approved. Resolution of the State Statistics Committee of Russia dated 04/06/01 No. 26 does not exist, therefore the contents of this article can be considered as recommendations, and not as a tangible requirement for exact execution.

Documents required for issuing a personal card; information coding

To fill out a personal T-2 card, the HR officer needs the following employee documents:

1) passport (or other identification document);

2) work book;

3) military ID;

4) document of completion educational institution:

5) insurance certificate of state pension insurance;

6) certificate of registration with the tax authority;

7) order for employment.

All other information is entered based on the information provided by the employee.

When encoding information in a personal T-2 card, you must be guided by a number of rules for filling out, defined by the Standard for automated systems HR management:

1. If incorrect entries or information not related to this filling are made in the coding field, the T-2 form is considered damaged and must be rewritten. Therefore, it is recommended to either enter exact codes or not enter them at all until the T-2 personal card is submitted to the archive.

2. If there are no entries or negative answers (“I don’t have”, “I’m not a member”), the encoded part of this detail remains empty. 1 3. When indicating dates, the name of the month is written in full, the year is written in four digits; for example, September 14, 2004

4. Dates are coded in Arabic numerals in the following order: day/month/year; for example, 04/23/1998.

Filling out sections of the form

The name of the organization is indicated in full, for example closed Joint-Stock Company"Energostroi". The abbreviated name is indicated if it is. enshrined in constituent documents. The OKPO of the organization is indicated on the side.

An employee's personnel number must contain no more than six digits. It is assigned to each new hire or employee and does not change with any movement of the employee within the organization until his dismissal.

The taxpayer identification number and the number of the insurance certificate of state pension insurance are indicated in accordance with the certificate of assignment of the TIN and the insurance certificate of state pension insurance.

In the "Alphabet" column the first letter of the employee's last name is indicated.

The nature of the work is indicated: “permanently”, “temporarily”, etc.

The type of work (main, part-time) is recorded in full.

Gender is indicated by the letter "M" or "F".

Last name, first name, patronymic are written down in full and legibly.

The date of birth is established on the basis of a passport or other identification document and is written in full in the text area, for example, “January 9, 1975.” In this case, the code is indicated as follows: 01/09/1975.

When indicating your place of birth, the following must be taken into account:

  • the length of the entry, including spaces and punctuation marks, should not exceed 100 characters;
  • districts and regions are written in genitive case, without commas, with the following abbreviations: city - city, village - village, region - district, district - env., region - region, village - village, district - district, station - station;
  • the words aul, kishlak, village, village are written in full;
  • the code of place of residence is determined according to the All-Russian Classifier of Objects of Administrative-Territorial Division. OK 019-95, approved. Citizenship is written without abbreviations. Recording of citizenship and its coding is carried out according to the All-Russian Classifier of Information on the Population. OK 018-95, approved. Resolution of the State Standard of Russia dated July 31, 1995 No. 412 (OKIN).

Filling out the "Citizenship" column

The languages ​​spoken by the employee are indicated in full without abbreviations. The degree of knowledge of the language in text form is indicated as follows.

Information about language skills

When encoding the column “Knowledge of a foreign language”, two codes are indicated, with the first indicating the language code, the second - the degree of knowledge of it.

For example: “English - read and translate with a dictionary” is written in the “Code” column as: 014 1.

Education is recorded and coded according to OKIN and the All-Russian Classifier of Specialties by Education. OK 009-93, approved. Resolution of the State Standard of Russia dated December 30, 1993 No. 296 (OKSO).

Abbreviations of words when writing the name of an educational institution are allowed, without distorting the meaning of the name. In the case where the name of the educational institution includes the word “name”, you should write “by them”; if this is followed by the word “professor”, “academician” or something else, then you should write “named after professor”, “named after academician”, etc. Orders are not indicated in the names of educational institutions.

All information about education (qualification, direction or specialty) is established on the basis of an education diploma.

The free lines in the education section are intended for the name and date of graduation of the second educational institution of higher or secondary vocational education.

If an employee graduated from two secondary or two higher educational institutions in different specialties, both educational institutions are recorded. Diploma qualifications are indicated in the following order:

  • for higher educational institutions - “bachelor”, “master”, “specialist”. For the qualifications "bachelor" and "master" the direction is indicated, and for the qualification "specialist" - the specialty;
  • for secondary educational institutions - “technician”, “merchandiser”, etc.

The profession (main and additional) is indicated entirely on the basis of the staffing table and the order (instruction) on hiring. It is coded in accordance with the All-Russian Classifier of Worker Occupations, Clerk Positions and tariff categories(OKPDTR). An additional profession is considered to be one that does not coincide with the main profession, but may be of interest to the organization.

Work experience (general, continuous, giving the right to a bonus for years of service, as well as the right to other benefits established in the organization, etc.) is calculated on the basis of entries in the work book and (or) in other documents confirming the relevant length of service.

Marital status is recorded and coded according to OKIN.

Marital status information

Only family members are indicated as part of the family, indicating the degree of relationship. Closest relatives are considered to be father, mother, son, daughter, brother, a sibling, a person in whose care the employee was, and other close relatives living with the employee.

Passport data is indicated in strict accordance with the passport. The residence address is recorded according to the rules for recording the place of birth.

In the event that an employee did not receive a work book on the day of dismissal due to absence from the workplace or for another reason, the personnel service employee will specified addresses sends notifications by mail about the need to come to the personnel department for a work book (or agree to send it by mail) or requests about the whereabouts of the employee. Information about hiring and transfers to another job is entered on the basis of an order (instruction) on hiring (forms T-1 and T-1a) and an order (instruction) on transfer to another job (form T-5). When filling out this section, the HR specialist must familiarize the employee with the entries made against receipt.

When filling out section. 4 “Certification” of the personal card, all columns are filled in, namely:

  • date of certification;
  • decision of the commission, for example, “transfer to a position,” “send for advanced training,” etc.;
  • a link to a specific document (usually a protocol of certification results) indicating its number and date.

The “Bases” column may be blank, or it may indicate an organization’s order to send an employee for certification and/or an order to approve the certification results.

Data on advanced training are recorded on the basis of documents submitted upon hiring or received from the personnel training department. In the section "Advancement of qualifications" the following is indicated:

  • start and end dates of training;
  • type of advanced training;
  • name of the educational institution (institute of advanced training of the ministry (department), faculty of advanced training at a higher educational institution, faculty of advanced training at a secondary vocational educational institution, institute of advanced training, advanced training courses at the ministry (department), advanced training courses at enterprises, scientific research and design and engineering organizations, higher and secondary vocational education institutions, institutes for advanced training and their branches);
  • type of document (certificate, identification);
  • the “Bases” column may be blank, or it may indicate an order from the organization to send an employee for advanced training.

Similarly, information about professional retraining is entered, indicating the specialty (direction, profession) in which the retraining is taking place.

When filling out section. “Incentives and awards” it is necessary to indicate the types of incentives applied to the employee (both at the organizational level and at the level of ministries and departments), as well as list state awards.

Vacation accounting

In the "Vacation" section, records are kept of all types of vacations provided to the employee during the period of work in the organization. The basis for making entries are orders for granting leave.

When filling out the vacation table based on an order, only the start date of the vacation is entered, while the end date is entered “de facto”, after the employee has actually returned from vacation. This is due to the fact that if an employee is called back from vacation or is interrupted, the “End Date” column will indicate the employee’s return to work date, and not the planned end date of the vacation as ordered.

Leave without saving must be noted in the same table. wages. It is taken into account when calculating the length of service that gives the right to annual basic paid leave, because according to Art. 121 of the Labor Code of the Russian Federation and it does not include the time of granting an employee leave without pay lasting more than 7 calendar days.

Similarly, the length of service giving the right to annual basic paid leave and the time of parental leave before the child reaches the legal age are not taken into account.

In the “Additional Information” section, for completeness of accounting, where necessary, indicate:

After an employee’s dismissal from the organization, entries are made in section. XI “Grounds for dismissal”, which indicates a breakdown of the grounds for dismissal of the employee in accordance with the exact wording given in the Labor Code of the Russian Federation, the date of dismissal, and the number of the dismissal order.

When closing the T-2 personal card, the HR employee puts his personal signature with its transcript and indication of the position. The employee himself signs in the same way. In this case, the employee’s signature confirms his agreement with all entries made in his personal card.

Changing employee information

When information about an employee changes, the corresponding data is entered into his personal card, which is certified by the signature of a specialist from the HR department, and if changes are made to section. “General information” and “Information on military registration”, then with the signature of the employee himself.

Changes are made according to the same scheme as in the work book. Based on the first forms and during the implementation of an automated personnel accounting system in the USSR, the OST "Standard for automated personnel management systems" was developed. It outlined the basic rules for filling out a personal card. As stated in the OST, “when a last name is changed, the old one is crossed out and a new one is written in its place in accordance with the documents submitted to the personnel department.” Further, “all subsequent changes to the employee’s data (change of place of residence, etc.) are reflected in the personal card by an employee of the HR department.” A complete replacement of a personal card is not advisable for the reason that in the future it will be difficult to restore the entire picture according to to this employee.

As for changes in passport data and marital status, the same scheme can be used - with a strikethrough.

Form T2 “Employee Personal Card”: sample filling

Let's show an example of filling out a T-2 employee’s personal card.

An employee’s personal card can be maintained in a form developed and approved by the employer independently, or in a unified form T-2, approved by Decree of the State Statistics Committee of Russia dated January 5, 2004 No. 1. If the employer maintains a personal card in a unified form T-2, he must enter contains all the information required by this form.

When registering a personal card in form No. T-2, information classifiers are used, which contain, in particular, codes for filling out the cells: OKIN (All-Russian Classifier of Information on the Population), OKSO (All-Russian Classifier of Specialties in Education) and others. Previously, coding was used to process data automatically. Now there is no need to encode information. If the employer decides not to enter codes, the code boxes on the card must be left blank. They cannot be removed from the unified T-2 form.

In the header of the T-2 form, you must fill in the name of the organization, OKUD, OKPO code. Next, fill out the table:

  • date of drawing up the personal card;

The main part of the card consists of 11 sections. In Section I you need to fill in general information about the employee:

  • employment contract (number, date);

  • family composition;

Section II “Information on military registration” contains information about the employee’s military rank, reserve category, etc.

In Section III “Hiring and transfers to another job” the following items must be completed:

  • the date from which the employee was hired by the company or transferred to a new place of work;
  • structural subdivision;

Section V “Advanced training” contains information about vocational education, received additionally by the employee.

Section VI “Professional retraining” contains information about additional professional education received by the employee. Typically, this section contains information about the training that the employee completed while working for the company.

Section VII “Awards, incentives, honorary titles” contains information about employee awards.

Section VIII “Vacation” contains information about vacations.

Since the personal card contains the signatures of the parties to the employment contract, namely the signature of the employee himself, confirming his familiarization with the information entered in the personal card, it must be on paper. You can maintain personal cards in in electronic format, but in this case there must be paper versions with the signatures of the parties.

The personal card is filled out for each employee, including part-time workers, in one copy. Abbreviations are allowed when indicating place of residence or information about education, but in a generally accepted form that does not distort the meaning of the information.

The completed T2 form must contain codes corresponding to the all-Russian classifiers: OKIN, OKATO, OKSO, OKPDTR. The number designation must be entered in a special table on the right side of the line. All code designations are freely available in printed reference books, as well as on the All-Russian Classifiers website.

If you fill out the classification code incorrectly, the T2 form is considered invalid, in which case you will need to create a new personal card. Strikethroughs and corrections in the coding table are not allowed.

Changes in the employee’s work activity or personal data must be reflected in a timely manner in T2. All changes made to the personal card are certified by the signature of a specialist from the HR department.

Information is entered into T2 on the basis of supporting documents provided by the employee.

The words and phrases “no”, “wasn’t”, “not available”, as well as dashes are not included in the T2 form - such lines are left blank.

Grounds for entering information about employees

The T2 personal card must be filled out based on the following documents:

Employment contract with the employee;

Orders on the organization: on hiring, transfer, granting leave, dismissal;

Passport (other document replacing it) of the employee;

Documents confirming the education received, courses completed, etc.;

TIN certificate;

Military ID.

Other information (for example, the degree of proficiency in foreign languages) is indicated only from the employee’s words. If necessary, the employer may request other supporting documents, in accordance with the specific professional orientation of the organization, as well as the established internal regulations.

How to fill out the title?

The first page of the form contains information about the employee and his level of professional qualifications. Filling out the T2 card (a sample of entering information is presented below) begins by indicating the name of the employer's organization in the top line of the form. The name is indicated in full, in accordance with the statutory documents of the organization. The coding table contains the digital designations OKPO and OKUD. Next, the following data is entered into the table below:

Date of filling T2 (Arabic numerals);

Personnel number assigned to the employee upon employment;

TIN (based on the Federal Tax Service certificate);

SNILS of the employee;

In the “alphabet” column the first letter of the employee’s last name is entered;

Completing this section begins by indicating in the “Employment Agreement” table the number and date of the employment agreement concluded with the employee. Next, filling out the T2 card, a sample of which is presented below, is carried out in the following order:

1. Line “1” indicates the full last name, as well as the first and patronymic of the employee.

2. In line “2” the date of birth is indicated in text form. Example: "April 20, 1981." In the table, the same date is encoded in Arabic numerals: “04/20/81”.

3. Line “3” must contain information about the place of birth. It is important to be guided by the following requirements:

When specified settlement the words “city”, “settlement”, “region”, “village”, “district”, “region” allow generally accepted abbreviations: “region”, “kr.” and so on. Example: “hor. 4. Line “4” is intended for entering information about the employee’s citizenship based on the passport. All subsequent information in this section is entered in strict accordance with the OKIN code. Please enter information as follows:

In addition, OKIN contains specific codes for foreign languages, which must also be indicated in the coding table. 6. The sixth line of section “I” is filled out solely on the basis of educational documents provided by the employee. An example of a text designation and the corresponding OKIN codes are presented in this table:

Further, in the table below, you should indicate: the name, series and number of the education document provided by the employee, the year of completion of training, as well as the qualifications obtained and the name of the profession in exact accordance with the document. The profession code according to OKSO is entered into the table. If an employee has provided two documents on education, for example, secondary vocational and higher education or first and second higher education, information about all available documents is entered in T2. Similarly, sections IV - VI contain information about retraining, certification, and advanced training received by the employee after employment.

Information about incomplete education should also be entered in T2, indicating the type of education available. For example, for an employee undergoing training at the time of employment, information about the education that he had before entering the current educational institution should be entered; based on a certificate from the educational institution, indicate that the employee is currently studying, the name of the educational institution and the course of study. In a similar way information is entered if the employee underwent training, but did not complete his studies, but was expelled (or was expelled):

If less than three courses have been completed, only the education that the employee had before admission should be indicated;

Information about completed training, subject to completion of three or more courses of an educational institution, is entered into T2 on the basis of a diploma (certificate) of incomplete higher education; in this case, the designation “Incomplete higher education” and the course from which the employee was expelled are indicated;

7. Lines “7” indicate the full name of the main and, if any, additional position (profession) of the employee, which he occupies in the organization in accordance with the hiring order and staffing table. Abbreviations or distortions of the name are not allowed. The section coding table indicates the job code according to OKPDTR.

Work experience

It is entered in accordance with the employee’s previous work activity, the basis is the work book. If an employee is employed for the first time, zeros are entered in the first line “general”.

Information about the employee’s relatives: who should be indicated?

Completing this section begins with indicating the employee’s marital status at the time of employment. Words and phraseological units “married”, “in a civil marriage”, “cohabiting”, etc. should be avoided: the line is filled in with the wording determined by the all-Russian classifier indicating the OKIN code.
If the marital status changes, amendments are made on the basis of a registry office certificate, indicating the details of the document.

Information about relatives directly living with the employee, indicating the degree of their relationship, should also be included in such a document as the employee’s personal card. A sample of filling out T2 looks like this:

Military registration

Information on suitability for military service is filled out on the basis of the provided military ID, indicating the rank and reserve category noted in the document. Information for items 3-4 is contained on page 3 of the military ID. For an employee who has not yet served, but has no restrictions on service, the lines are left blank, with the mark “subject to conscription” being entered in line 2.

The degree of suitability for service is indicated on the basis of the registration certificate by the letter designation:

Fit with restrictions - B.

Limited fit - V.

Temporarily unfit - G.

Line 8 of the section is intended for the marks “Removed from military registration” indicating the reason, for example, due to age or health status.

Page 3, section III

This section exclusively contains information about the transfer of an employee to another structural unit within the same organization (including transfer to another location) or about a change in job functions. All information is entered on the basis of orders for the organization, indicating the details of the order (date, number).

Important to remember! Information about changes in rate or salary made without changing the employee’s position should not be indicated in this section.

The employee must be familiarized with all entries made in this section against signature.

Employee holidays and rest days

This section is filled out as the employee is granted leave in accordance with labor legislation and internal regulations. In the section table you must enter the details of the vacation order, type of vacation, and number of calendar days. For annual leave, you should also indicate the period for which the benefits are given. vacation days, in columns No. 2, 3 of Table VIII (employee’s personal card). Section X: What information can I provide?

“Additional information” is a section provided for information that may be related to work activity, but is not indicated in other sections of the T2 form. Such information may include, for example, whether the employee has driving experience and a driver’s license of a certain category, health restrictions (in accordance with a medical report), etc.

How to correctly enter information about dismissal

The last section is completed on the employee’s last day of work. You should indicate the details of the order, the reason for the employee’s dismissal and confirm the entry made with the signature of a personnel employee. The employee must also be familiarized with the records of Section XI by signature.

When a decision is made that a candidate for a vacancy will work at a given enterprise, and an order to hire him is issued, it is advisable for HR department specialists to create a register of the employee’s personal card. Although it does not apply to the mandatory documents that must be issued to the employee, in some cases the law requires its presence.

When joining an organization, an employee submits a certain package of documents, which is filed in his personal file. To summarize and collect all available information about him in one form, and subsequently reflect information about his work, the employee’s personal card is used.

Moreover, if this form is filled out at an enterprise, then the card must be issued in the personnel department for each employee employed both at the main place of work and part-time.

In addition, it is a register, which is absolutely necessary for the purposes of military registration at the enterprise. Specialists of the military registration and enlistment office monitor not only the correctness of registration, but also the completeness and relevance of the information contained in it.

It is advisable to designate a person responsible for these purposes at the enterprise. It should also be taken into account that the card contains personal data that is subject to protection at the enterprise in accordance with the law.

Attention! Periodically, the employee must check the information recorded in his personal card and confirm its correctness with his signature.

Rosstat provides a standardized personal card T 2. The enterprise is given the right to create its own personal data register on its basis.

Most software packages use a unified form that is filled out automatically based on data previously entered into the program.

The employee card, both in paper and electronic form, must be stored in a secure place with fixed access. At the same time, only persons approved by the manager should have access to it.

Important! After the dismissal of an employee, the T-2 form is transferred for storage to the archive service of the enterprise, where, in accordance with the law, it must be kept for 75 years.

What documents are used to fill out a personal card?

All information contained in the card is entered based on the collected information. They can be provided by the employee, or issued at the enterprise.

On the basis of what documents can the T-2 employee card be filled out:

  • The main document that should be reflected on the card is.
  • Passport.
  • SNILS.
  • TIN (at the same time this document does not apply to mandatory requirements for employment).
  • Certificate or diploma of education.
  • Military ID.
  • Data about marital status are filled out on the basis of the information contained in the document previously completed by the employee (if available) or copies of marriage certificates, birth certificates, etc. provided by him.
  • The HR officer can take work experience from labor worker, and in its absence (part-time work) on the basis of a certificate submitted by the employee from the main place of work containing this information.

Attention! In the future, it is necessary to enter information into it based on orders for admission, leave, transfer, promotion, dismissal, etc. Moreover, the data from the order for admission, transfer, dismissal must be confirmed by the signature of the affected employee.

First, fill out the header of the form. The full name of the company and OKPO code are recorded here.

Then the date of registration of the document, the assigned personnel number, TIN and SNILS codes are recorded. Next, enter the first letter of the last name, the nature of the work, the type of activity (main, part-time), and the gender of the employee in the form of a letter.

General information

First, in the two columns on the right, the details of the employment agreement concluded with the employee are indicated.

IN columns 1-3 Full name, date and place of birth are recorded. In the columns to the right of this information, codes from reference books are indicated.

IN column 4 citizenship is indicated.

Information about military registration

Information is entered into this section only for male employees based on data from a military ID or registration certificate.

IN column 1 the inventory category is recorded. It is not filled out for officers.

IN column 2 the title is entered. If an employee is just going to military service, then “subject to conscription” is entered in this field.

IN column 3 the type of personnel is indicated - medical, soldiers, etc.

IN column 4 The code of the acquired specialty is recorded.

IN box 5 The letter from “A” to “G” indicates the degree of suitability for military service.

IN box 6 The name of the military registration and enlistment office where the employee is registered is indicated.

Hiring and transfers to other jobs

Card T2 contains information about the employee’s movements between structural divisions companies.

The date of transfer, the name of the unit and position, information about the salary at the new place, information about the order according to which the move was carried out are recorded here.

The last column is the employee’s personal signature, which he puts in confirmation of familiarization with the information entered.

Certification

The results of certification at the enterprise are recorded here. The first column reflects the date of the event. Next, the decision of the commission on this employee and information about the certification protocol are indicated.

The last column is the basis; it can remain empty, or the data of the order by which the decision was made to carry out certification in the company is entered here.

Training

Information about the fact that the employee has completed advanced training courses is recorded in the employee’s personal card. The first two columns record the dates during which the courses were taken.

Then the type of requalification, the name of the educational institution where the courses were taken and information about the document confirming the completion of the courses. The last column is the basis for retraining, usually this is data on the order according to which the employee went for training.

Professional retraining

Information about retraining is recorded here. The period of retraining, the name of the profession for which it was carried out, information about the document issued to confirm the training and the basis for the referral for training are indicated here.

Promotions and awards

This includes all types of awards that an employee receives while working for the company (bonuses, certificates, valuable gifts, etc.). The name of the incentive is written in the first column, and the basis (data about the order that established the fact of issuing the incentive) is written in the subsequent columns.

Vacation

All facts of providing him with rest time are recorded in this section of the employee’s personal card. At the same time, absolutely all vacations must be entered here - primary, additional, educational, etc., regardless of their payment.

First, indicate the name of the vacation period. Next, the periods of working time for which it is provided are recorded (here, for example, the working year for which paid leave is issued is entered), the duration of the rest and the start and end dates of the rest.

The last column is the basis for leave; the data of the order according to which the employee is granted leave is recorded here.

Social benefits

Information is recorded here when an employee is provided with working conditions that improve his situation. For example, if an employee has a group 3 disability, then by law he is given a shortened working day.

In the table you need to indicate the name of the benefit, then information about the document that confirms it. The last column records the basis for the provision. Here, for example, you can enter the details of the regulatory document that establishes such a benefit.

additional information

This includes any information that cannot be included in any of the previous sections, but is very important. This may include, for example, the presence driver's license. In this case, the document number, category and validity period are recorded in the section.

Grounds for termination of an employment contract

This section ends the preparation of the document form T-2. Information is recorded here when an employment agreement with an employee is terminated.

When entering information, the personnel officer must write down the reason for dismissal, the date, as well as information about the order (date of its execution and number). Having done this, the personnel employee checks the correctness of the data entered and confirms this with his personal signature.
